PRODUCT DESCRIPTION

Thinking about how to prepare for your full evaluation this year (or in the coming years)? Want to provide your administrator with an organized, professional-looking, thorough record of all you have accomplished?

Use this fully-customizable template throughout the year to document your proficiency with each of the required competencies and never forget to mention a skill or fantastic teaching success again!

The document contains two columns and bulleted lists within each ("Things I've Done" and "Artifacts I Have") to satisfy each competency and sub-category. At the bottom of each section, there is a designated area for additional notes regarding that competency.

This zip file contains two of each: a Word 2011 (version 14.3.7) document and a PDF document. One version is completely blank and one includes a few generic anecdotal examples to get you started. Using Adobe Reader or Adobe Online, the PDF is customizable by adding text, and using Word the file is fully customizable.

The file was created to align with the teacher evaluation competencies in Loudoun County Public Schools, Virginia, but could easily be customized to align with any school division or state.
