This form can be used to plan and prepare for parent conferences.
This form would be given to any teacher that works with a particular student, for written input on students’ behavior and academics. This form is particularly helpful if a teacher is unable to make it to a group meeting, but needs or wants to contribute information.
As the Learning Resource Teacher at my school, I help annual review parent/teacher meetings for all students on academic support plan or students that were on intervention. I gave these out to teachers before the meeting were planned to let them know when it was planned and ask for their input. I was able to review their comments and prepare for the meeting and if a teacher was not able to make the meeting, I was able to show that the teacher has contributed valuable information.
Parents appreciate such preparation and seeing that all teachers that work with their child is vested.
