Our teachers are often unaware of the wealth of information they are able to give us to help us determine the best placement or goals for our students. I can't tell you how many times I talked with teachers this year to get their input and in return got blank looks or, "Well, their speech is just really behind." What does this even mean?!
We also constantly need information about a student's performance in the general education curriculum... math, reading, writing. We need to know if they are receiving RTI (MTSS), if they are succeeding or struggling, etc.
This form gives teachers examples and other statements step-by-step to help direct their input and narrow down their concerns. We need this type of information throughout the school year for a variety of purposes:
-Evaluation planning and report writing
-and so on!
