This is a versatile and easy-to-use resource that can be used for teacher observations of any course subject. Administrators could use this for evaluative purposes, but it could also be used for peer observations if you'd like. The form is a 4-page editable word doc that can easily be changed to fit your unique needs and purposes.
There are three main parts to this observation form. The first page is focused on the area of "relationships and school culture," and the scoring criteria can be seen in the preview. The second page is focused on "instruction and rigor," so that page will be more centered on the learning and engagement seen in the classroom. The third page is focused on "classroom management."
Each of the these three categories has six specific questions that will be scored by the observer on a scale of 1-5. At the bottom of each page, the scores of each category will be tallied up to be a maximum total of 30. They are scored separately in order to quickly see areas of strength or development.
On page 4, the teacher will have a comprehensive score out of a total of 90. There is also space to provide notes and/or meaningful comments, write questions or points of confusion, and also to write down any next-step and/or support plans that need to happen. There is also space for each party's (observer and teacher) signature at the end of the document.
