Teacher Recommendation Letter Request Forms

- PDF

Description
For survival-mode seasons when you have teacher recommendation/reference letters to write in addition to all the other things, these request forms will give you insights into your students that will help you write memorable recommendation letters.
In this PDF download, you'll receive the following:
- one blank Teacher Recommendation: College Admissions Request Form (also suitable for scholarship reference letters)
- one blank Teacher Recommendation: Programs/Activities Request Form (appropriate for apprenticeships, internships, camps, summer travel programs, early college experiences, etc.)
- one completed model Teacher Recommendation: College Admissions Request Form
- one completed model Teacher Recommendation: Programs/Activities Request Form
These forms allow students to provide content ideas to give an authentic picture of themselves as individuals. While you don't have to tell the same stories or even incorporate the same traits they write on the request form, reading their ideas will jog your memory and help you reach the next level in your recommendations. Well-written recommendation letters are increasingly important as programs and funding continue to be more competitive.
To help your students provide more specific information on the forms, consider presenting the models in class.
These forms work best for high school grades.
NOTE: The school where I developed this request form used an online college planning and admissions support system that corralled the application process on their website. We did not have to track when/where to send recommendations ourselves. If your school does not use a service like Naviance or Scoir, this resource might not meet your college recommendation letter-writing needs.
