Used when working 1:1 with a student on specific tasks by category (literacy, math, writing, building/leisure/fine motor). This sheet can be edited for any category task that works with your student. Measure independence/supported and if supported what type of prompting is needed. Measure the number of times a student attempts a task and take notes on specific details for accommodations needed.
This is the first assessment tool. Once students have mastered a task, you can use the "independent work" data sheet to monitor if they are able to generalize the task from one station to the next, or if they needed additional prompting once a teacher was not in front of them.
