This is a CCSS aligned parent and teacher rating form that can be used during evaluations and before annual IEPs for students in Kindergarten through 5th grade. The teacher forms are especially helpful for teacher collaboration. It's an easy way to show and explain to teachers how you can help their students meet specific CCSS. These forms allow you to gather input from teachers so that you know exactly what areas the child's teacher feels are most impacted in the classroom.
These forms also help you build rapport with parents and provide them with a more active role in the IEP process. This form allows you to gather input from parents in a way that puts everyone on the same page: the form uses parent-friendly language while matching common target areas to CCSS categories. It will help you understand how the parents perceive their child's skills at home and what areas they see as the most important to remediate. This gives you a broader view of the child's skills outside of the speech room while helping you build rapport with parents and teachers. Once you know what breakdowns are most important to teachers and parents, you can target those areas, and more progress will be noted outside of the speech room. Win, win!
There are two forms: one for teachers, and one for parents. Both the parent and teacher forms are split into lower and upper elementary grades: one form for Kindergarten-2nd, and one form for 3rd-5th. Each form breaks down common speech & language targets into the following CCSS categories: Language, Speaking & Listening, Reading Literature, and Reading Foundational Skills. For each target within a category, the teacher or parent is asked to rate the child's performance on a scale of 1-5. After all targets are rated, the teacher and parents are asked to choose which three targets they feel most impact the child at school and home.
