As an itinerant Teacher of the Deaf and Hard of Hearing, I have some students on my caseload who I only see once a month as a consult. It is important to keep track of these students who are mainstreamed and consult with their classroom teachers to ensure they stay on target and receive appropriate accommodations.
This checklist can be used all year. It keeps track of amplification, FM functioning, background noise, seating, academic targets, social skills, self-advocacy and more. Also includes space for monthly written notes.
