*Welcome Back Attendance Sheet- Easy to use sheet that allows the teacher to track the students attending the first day of school (those on the attendance roster, those new to the school) and boy/girl count. This sheet is set up for either a class of 20 (1st page) or a class of up to 33 (2nd page).
*Field Trip Info. Tracking Form- This field trip tracking sheet was created to make the field trip experience a little less stressful. It includes areas for: the student's names, permission slip status, lunches (bringing or buying), student emergency phone numbers, a spot for notes and reminders, student's special needs or medications, the destination name/phone #, contact person's name, arrival time, principal and school's contact information. This sheet is set up for either a class of 20 (1st page) or a class of up to 33 (2nd page).
*Parent Information/Signature Card & Contract-
These cards really come in handy!! I copy them back to back on bright colored card stock, cut them in half and keep them next to the phone. I like the students to know that I can get in contact with their parents at any point in time (notice the separate areas for mother and father info., this helps when students have parents that are divorced). The Classroom Contract side, serves as a reminder to the parents. They must check off that they have read and understand the following: 12 Steps to Being a Successful Scholar (SEE MY OTHER PRODUCTS FOR A COLORED COPY OF THIS ITEM, PLUS EACH STEP ON AN INDIVIDUAL CARD), student behavior information, homework information and criteria, and the student planner requirements. Great source for getting in touch with parents, as well as verifying signatures on the important papers that students return.
*12 Steps to Being a Successful Scholar- This is a must have!! This sheet hits all areas for students! The focus is on being a scholar (incorporating several GATE strategies). Areas covered: BE CONFIDENT! BE RESPONSIBLE! BE HERE! BE ON TIME!. BE FRIENDLY! BE POLITE! BE PREPARED! BE A LISTENER! BE A DOER! BE A TOUGH WORKER! BE A RISK TAKER! BE A GOAL SETTER!

*Parent Contact Log- We all know how important it is to document the parent contacts that take place throughout the school year. This is a ready to use sheet that can be placed on a clipboard by the phone or whole punched and placed in a binder.

*Cafeteria/Lunch Rules- This list was created with the help and input of students. It includes several basic, common courtesy rules that students should follow when in the cafeteria.
*Check List Slips (3 different formats)- These are the slips that I use each and every year without fail!! I use them to check homework, track project progress, to monitor who has/hasn't returned important papers and even to jot down who is bringing what for a party. I like to copy these on bright colored paper and cut them into slips (a different color for each trimester). For checking and tracking homework, I get one of those big binder clips and keep them all together (really helps when a parent calls and wants to know which assignments their child is missing).
*Know the Rules (Cafeteria/Recess/Playground/Bathroom)-
This sheet covers the appropriate behavior that is expected from students. There is a spot at the bottom of the page for the student to sign, documenting that they agree to abide by the rules.
*Professional Growth Tracking Sheet- Keeping track of your professional growth is important. Although you can renew your credential online, you still have to have documentation showing that you meet the required hours and/or units in the different domains. I have also found that this sheet comes in handy when reviewing my yearly professional goals during the evaluation process.
*Divisibility Rules (rules and example)
*Prime or Composite Activity- These two math pages have been created to help students understand what can seem like a difficult concept. It shows students the rules for divisibility and also provides examples. Then the prime and composite page takes students through the process of finding prime numbers, it also allows students at all levels to be successful.
