Teletherapy Assessment Reminder Checklist for Parents and School Facilitators

- Zip (2 pages)

Description
When doing a speech-language assessment virtually (via teletherapy), it is critical to prepare the parent ahead of time. Prior to the assessment, send this checklist to students' parents to give them information/reminders for how to help the evaluation go smoothly.
The checklist provides information on...
- Technology set up
- Creating the ideal environment for testing
- Reminders for the parent's involvement during the evaluation.
There is also a second copy of the checklist, which is geared toward a site facilitator (such as a para or other school staff member) if the student you will be testing will be at a school building and not at home. At the bottom of the checklist there are editable spaces to type in the SLP's name and contact information.
The downloaded file is a zipped file, containing both versions of the checklist/letter (parent and school professional), plus a full version with credits, terms of use, etc.
