There are a few essential things we need to know before a child is dropped off that first day of school. Here is a parent form (in Spanish) you can use at Meet the Teacher night, Kindergarten Beginning of the Year Assessment Night, or at Open House Night. If you miss those three opportunities, you can also send it home on the first day or week of school.
It has requests information such as:
1. Parent name
2. Parent telephone numbers
3. Transportation information
Plus, it requests other information that is crucial for us teachers to know about every child - key information that we may forget to ask in the crazy chaos of the first few days. Some of this information is vital, information such as:
1. Does your child have any allergies?
2. What motivates your child?
The more you know, the more you can connect with the child. The more you connect with the child, the bigger the trust they will have in you to provide a safe, nurturing environment. Plus, your parents will feel secure in leaving you their most precious child.
NOTE: This form is in Spanish and is to be used in Spanish, Bilingual, or Dual Language classrooms.
