This template can be edited to discuss the details of each student you are assessing. The template was created in order to make the assessment report as easy as possible for the assessor.
Replace the existing name Sam, within the report, with your student’s name. There are no pronouns within the document so it can be used for either male or female students without needing to change any pronouns.
The scoring table within this template (Meaning of Standardized Scores) is used to state the range where students score within each test. The template states the student scored average for each test so the assessor needs to either leave the word average or add in the words below, low, high, superior or very superior before the word average. For example, if a student’s standard score on the test is 112, the scoring would be considered high-average for that test. The word high can be added before the word average in order to describe the student’s score on that test. If a student’s standard score is 75 on a test, add the word below before the word average for that test. Within the table (Woodcock Johnson IV Assessment results) the level is also stated as average for each test. The words below, low, high, superior or very superior can be added before the word average to accurately reflect your student’s score.
