The Test Correction Form for grades 3-12, adult education, and homeschool can also be found in the Formal Assessment Bundle
If you are looking for a way to get students to learn from their mistakes, take ownership of their learning, analyze and critique their own work, and require them to learn material even if the test is over, then this test correction form is one way to do it.
Use this form to help students analyze and re-try incorrect answers from graded tests. You can make this a requirement to re-test, adjust test scores based on their corrections, make it homework, or part of an in-class exercise.
Students will complete a simple table where they retry the incorrect answer and then answer questions that require the student to evaluate their old response and justify their new work as accurate.
This test correction sheet is versatile enough to work in nearly any subject area. I have used it with my math students for a few years now and wouldn't consider handing back a test to a student without it.
