It is imperative that students analyze their performance on assessments and reflect on why the answers they selected were incorrect. This is a basic test analysis template for students to use to learn from their mistakes and have one final opportunity to master the learning objectives or learning targets being assessed on the test. By reflecting on why their answer was wrong and how the correct answer is in fact correct, the students are learning the material. The teacher is also able to see why students got particular questions incorrect, which will inform and improve his or her teaching.
I use this form after the graded tests are given back to the students. They go back through the test questions and reflect on every incorrect answer using this form.
