This is a checklist that contains several different types of accommodations a student with an IEP may use on a test or quiz. I attach one of these to every test or quiz to let the parents and the homeroom teachers know what accommodations were used.
There is also a spot for teachers to record how many times the test or quiz was checked; allowing the student to go back and check it. I typically only check it one time; giving the student the chance to go back and make any corrections.
I have these copied on NCR paper so that I have a copy for my records, which I then summarize in Present Levels of the IEP. Parents and teachers really appreciate this checklist of accommodations.
