One of the joys of teaching at most schools during the first few days of a new year or semester is distributing textbooks, logging numbers and conditions, and - conversely - evaluating the same at the end of the course. What a pain! If you have ever had a book returned in questionable condition after having spent months in "who knows where" only to have the returning owner argue that same book was issued to him with a praying mantis slammed between pages 340-341... well, you understand the need for a more definitive method of evaluating wear and tear. Hence, the Textbook Condition Sheet - a sheet completed by the student upon issuance of the book, retained by the teacher through the course, and compared again the return condition of the text at the end of its use by that student. Ideally, the teacher might retain the paper to check against a new condition sheet filled out on that same book by a different student next semester/year. Please feel free to modify or adjust this sheet as needed for your purposes!
