My co-teacher and I had a class that seemed fascinated with signing out to the bathroom. I don't know what was going on in the bathrooms, but apparently it was more interesting than what was going on in our classroom! It was an all-day pursuit for some of them. That's when I came up with this sign out. The DocX document has 21 place holders so you can type in each of your students' names. There are identified areas for them to sign out ONCE in the morning and ONCE in the afternoon. Then we hung a clipboard on the wall and put a fresh sign-out sheet up each day. With the clipboard on the wall we could easily see if the students were actually signing out each time. We also stuck a large pencil next to it with a hook and loop dot. You will need a digital clock for younger students and you'll have to show them how to put the hour on the left hand side of the colon and the minutes on the right. They were instructed to put one line through the time when they returned to the classroom.

I've also made passes that are meant to be used for a month and then thrown away. I've never seen a commercial bathroom pass last the whole year and look sanitary doing it. These are decorated with monthly themes featuring animals and monsters. Print, cut and then laminate. Punch out the purple dots and cut lengths of yarn about 28 inches long to string into each pass. Have the students wear them around their neck for when they use the bathroom. At the end of the month THROW THAT PASS AWAY! You'll feel cleaner doing so. I've included some extra passes for the hall and nurse's office, and even a generic bathroom pass for when one goes missing.

The passes are provided two ways: An editable 5 page PowerPoint version so you can add your name, and a 5 page PowerPoint without a name place holder in case you just want to print it right away.

Tip: Cut the passes before laminating and leave a sixteenth of an inch laminate border so they don't peel apart. To make them even more sturdy, print them on card stock. Alternately, paste them to card stock, leaving a nice border. Coordinate the card stock colors if you're as excited about color combinations as I am.
