I designed this to be quick. Often parents of a child with special needs want to know how that child's day was and teachers find it difficult to write notes out in a communication book. The end of the day is just too hectic. On this chart you fill in up to 6 behaviors which you want to be able to tell the parent/guardian about. Then you mark 1, 2 or 3 checks depending on how often you saw that behavior. You also mark 1, 2 or 3 checks to show if the child needed help with those same behaviors. A quick glance allows you see where the child was successful and where they needed help. Work with the parent to choose behaviors on which to report and emphasize the positive.
