This "think sheet" makes it easy and effective for students to reflect on their choices. If a student makes a poor choice, they can write/draw what they did wrong, then write/draw how they could have solved the problem. It then has spaces for teacher and parent comments and signatures.
There are two different options for printing this Think Sheet. In my classroom, we use a color coded classroom management approach. (The kids start on green, then can be moved to yellow, orange, or red if they make a bad choice.) The first page has a spot for the kids to circle if they were on yellow, orange, or red for the day. The second page doesn't have this on it, in case you use a different classroom management approach.
