I used this with great success last year. I have students use this Thought Tracker as a bookmark. Each day, after reading, I have students make an entry on their Thought Tracker. Some days I ask them to write about characters, setting, conflict, etc. Some days I ask them to make a prediction. Some days I may ask them to just jot down some of their thoughts.
Students turn in their completed Thought Tracker at the end of the week. I review their entry, write them a quick note and place the bookmark on their desk (along with a new one) on Monday mornings.
The Thought tracker can be easily edited. Sometimes I change the days to "Page #" and ask the students to summarize the pages they read that week.
A bonus is that there are 5 entries (Mon - Fri) which you could use as 20 points each to mark a nice 100 point grade if needed.
