This is a chart I developed for collecting timed-interval data from classroom observations of students. The chart can be used with any interval of time, and is used to determine time on-task and off-task, and other common off-task behaviors such as vocalizing, fidgeting, playing with objects, and out of seat. It also tracks when a student gets redirection, or one-on-one support from the teacher. There is a line to add a specific behavior you might be tracking. You can use the full chart to collect data on one student, or use half of the chart for the identified student, and the other half of the chart for a "comparison student". Upon completion, calculate the percentage of time the student spent on (or off) task during the time period. I have included a sample chart for reference.
