How do you accommodate your lessons for all of your students? This form can be used to track modifications and accommodations for your students in Special Education or on ELL plans. Many administrators are requiring that teachers show proof of lesson accommodations and this can easily be added to your weekly lesson plans and your student folders. This is also documentation you can use at your parent conferences. I would also suggest that you keep these for IEP meetings. As a special education teacher, I would find it beneficial to know what modifications are or are not working in the regular classroom when writing their IEPs. I have also included a weekly BIP tracker. You can write the area of concern and how they performed each day. I usually use a sticker with my students. I also require that the parents write their initials each day it is sent home.
