I use this activity after extensive study in all three linear forms. The students are to translate, convert, manipulate, or whatever you want to call it each linear equation into each linear form. This is a difficult skill for my 8th grade Algebra I students so any practice is good practice!
I've used this as independent practice, Think-Pair-Share, group work, and centers. All seem to be effective. Sometimes I add a line so that students can create their own problems. They enjoy that!
NOTE: In order to translate into Point-Slope Form, student must have a good working knowledge of factoring and greatest common factors. I find this must be attended to before the student can work without the teacher.
