This progress monitoring graph is used for students to graph and monitor their progress on reading tests for the Grade 5 Treasures program by McMillan/McGraw Hill. It is designed as a triple line graph where students can map their progress on the Weekly Assessment, Selection Test, and an averaged score listed as "Overall Score."
There are lines for each test below where students can notate the areas for which they need improvement. These areas can be given to the students via teacher comment, or the students can take them from the back of the Weekly Assessment as well.
Week to week, students can use this to reflect, analyze, and write about what led to increases or decreases in their scores as well as the behaviors, actions, or practices they need to keep or start doing to be successful.
