This is an editable choice board and a truck self-monitoring sheet for on-task behavior. The truck format of the self-monitoring sheet is great for students with autism that likes trucks (or any kid that likes truck!). The student can put a check mark or x in each box of the truck bed to self-monitor. This can be used with an interval time for consistent self-monitoring, or throughout a student's day based on when a teachers tells the student to self-monitor. A goal should be made before self-monitoring (e.g., 80% on-task) for which they can earn a prize off the choice board.
