This worksheet would be most useful for upper elementary. It allows students to think about themselves as a reader. It's great for a first week of school activity. I use it after every reading unit when I confer with students to see if they have changed as a reader and if they have new goals. Students can write inside the boxes or use sticky notes to interchange each month, unit and/or quarter. Students can keep it in their reading folder or turn it into the teacher to keep on file.
