This is the Unit 5 scale test which I give students after we have practiced writing scales. This test has eight major scales total, four in treble clef and four in bass clef. I always allow students to use a piano keyboard handout, though it's up to you if it can be one from their notes (which is potentially written on already) or if you hand out a new one for the purposes of this test.
You can specify what you require for this test: I personally only required the notes themselves (with appropriate accidentals); you could, however, also require note names written out, or the WWH formula.
Additionally, I usually gave out this test twice: the first time they could use their notes, and the second time (a few weeks later) they can't use their notes and they have to make the appropriate key signature.
I personally made each mistake count as 2 points, but the grading is up to you of course!
Finally, there are lots of ways that a student can check their work:
-check the letter names: one of each letter
-make sure the top note is the same name as the first note
-no mixed sharps and flats
