I designed the Unit Progress Tracker for students to keep their own record of their progress during a unit of study. The tracker includes space for students to write each assignment, their score, their effort level and indicate if they need help with the assignment. The bottom of the front page is for students to create a graph showing their progress during the unit. The goal is for them to see the correlation between positive progress on assignments and mastery of unit content.
The back of the tracker is for daily student progress reports. Students are responsible for writing the assignment/objective for the day and stating their progress towards completion. They also rate their effort for the day. I have students complete this section during the last 5 minutes of class.
For effort rating, I use a 1-10 scale with 1 being the lowest and 10 being highest. However, you could use any rating system you choose.
