I created this written assessment specifically for 4.NF.6. It is a 10 page assessment which includes a rubric to be used to assess the students from poor performer to proficient performer. There are four levels of expectations, each with a description. Also included is a sheet where you can record your students name and how they performed on the assessment task.
The assessment itself is 3 pages long. It begins by asking the students to design a pattern for tenths and for hundredth which they then convert to decimals. They are also asked to use number lines to place decimal fractions and decimals. The number line only include 0 and 1 and half is marked without being labelled. The main part of the assessment asks the students to use division to change the decimal fractions into decimals and all working out is required. Later questions ask the students to explain their thinking as well.
Answer key is included though students explanations may vary.

Whilst my resources have specifically been designed for 4th Grade, most will also be most applicable to higher ability students in grade 3 and lower ability students in grade 5.
