This is a form I developed to help guide students so they would know what questions to ask while going from station to station. It is specific for a vehicle career day, so they first write the type of vehicle, like fire truck. Next they ask the person their job title. Where stands for where do they perform their job. For instance, fire fighters would go to the fire location. What represents what the person specifically does on the job. The next two lines are for anything the student wants to ask or found interesting that they want to write down. This can be a graded activity where you give students credit for the work they completed. It can also just be used as a guide to help them stay focused during the event. I like to give a treat if their sheet was completed correctly and they remembered their manners. You could also choose to have students write how the job uses math and/or science on the bottom two lines.
