Studies show that when a child or individual can ask for they want, they are less likely to engage in problem or challenging behaviors in the classroom and at home.
Use this data sheet to track the number of spontaneous requests your students makes (e.g., breaks when work is difficult, assistance when they need help, missing items, attention, activities they prefer....etc.) as well as how many requests were contrived or prompted during the teaching process. This data can be graphed to demonstrate to determine if treatment or your teaching approach is working by comparing the number of prompted, contrived correct, and spontaneous requests your students emits per teaching session. Analyzing data and graphs is an excellent way to improving your classroom or therapy sessions.
Both the "Mand/Request Data Sheet" in PDF and the formatted graph in excel are available in this download.
