This is a tool that I use with my students when they read independently. Students must always have this form and a dictionary along with the book that they are reading. In addition to increasing vocabulary, this form promotes using context clues and dictionary skills. Each time a student comes across a word that they are unfamiliar with during independent reading, it must be entered onto this sheet. Forms are turned in once they are complete or the book is finished. (NOTE: one book may need several forms). I also like using these during group discussion and allowing students to share their new words with their classmates.
