This form has been proven very useful in helping to teach vocabulary over the years, and has been requested by a number of teachers. Students have clearly-marked spaces for the entry word, the word's definition, a synonym, an antonym, a clue word and even a picture to help them make those all-important synaptic connections.
A unique feature of this item is that a student self-assessment for learning each word is included. Students use a zero-to-five point scale to better gauge their familiarity with the words they are learning, and they can update their scores as their learning progresses. Very motivating!
Some teachers print these pages in various colored paper for different subject vocabularies such as reading, history, science, and even math. Other teachers (like me) bind a whole year's worth of blank pages together in a "Vocabulary Notebook" that has every word we've studied by the end of the school year. Quite impressive at Open House!
