Both of these documents are wonderful for enhancing vocabulary instruction.
The vocabulary log allows students to record any interesting, new, or unknown words they come across in their reading. It asks them to write down the word, the date they found it, where they found it, and what they think it means.
The vocabulary self-awareness chart allows students to assess themselves on class vocabulary words. In my own classroom, we choose our words from the words students have come across in their reading. Students write the vocabulary words, determine how well they know the word (noting it with a symbol), and write down what it actually means and an example.
