I made this center log for my students to use each week. This is helpful for them because it allows them to take personal responsibility for the choices they make during their literacy center time. It also allows me to see their preferences and which areas they tend not to work on as much (so I can guide them better or tweak the activities available to make them more interesting to them). It also helps me, because our school has moved towards grading work effort, so this gives me a way to assess center work on a point system so I can convert it to a weekly grade.
I have recently updated this product to include boxes that enable you to indicate how long students are taking to complete a task. Most of the time it isn't an issue, but we all know that occasionally we have students who are easily distracted and need a lot of support to see a task to completion. So, I added 3 boxes beneath every choice, each day. If students complete during center time, they earn a smile or star in the first box. If they need an additional amount of time, they earn their smile or star in the 2nd box. If they still need a third block of time, they earn their smile or star in the 3rd box. I also added a section where you can indicate whether or not the child was in a reading group with you that day.
11/3/13: An additional update has been made. I have added logs to use as you are training your students one center at a time, building up to all 5. This makes it easier for your kids to learn how to use the log without being overwhelmed by the choices they can't make yet. There is a "blocked out" page for each week as you train your kids. I also added a small square in each selection. This allows you designated space to write in how many points the student earned for the center.
