This resource is designed for students who need extra behavioral support. At the beginning of the week, have the student set a goal. For example, it could be... "This week, I will not interrupt my classmates." Or, "This week, I will not put my hands on anybody for any reason." The possibilities are endless. Set a goal for the amount of points they should strive for by the end of the week. Increase that number as behavior becomes better. The incentive can be a prize or reward. Send this sheet with them all day and have their teachers circle the number that best describes their behavior. Another option is that the students must take it home on Friday and have their parents sign it.
