I use this in my classroom every week. I type in the student name and the date (which you can add with text boxes on this PDF). Each student must keep this form all week. They mark when they break a classroom or school rule in the center cell of the top row of the table by writing a brief description of their actions. They include the date at the far right and cross out the grade on the left. "Above and Beyond" behavior does not raise a student's weekly citizenship grade, but will earn a small reward. At the end of the week, each student writes his/her final citizenship grade in the top right box. Parents sign the bottom of the form and students return them on Monday.
