Use this chart for students who need extra behavior support. Instead of "changing his color" or "flipping her card" right away, the student has 3 warnings. For the 1st warning, cross off the first colored circle. The student then has 2 more chances. When all 3 circles are colored or marked, the student "flips" or "changes" his/her classroom card (or moves stick, etc.). Keep this paper throughout the week for documentation. The student or teacher/ assistant can mark the circle. If the teacher/ assistant marks the circle, he/she should explain why the circled is being marked and explain to the student that he/she has # of warnings left.
