Students use this chart to first make goals for the week. Then, they refer to the chart daily to document what they accomplished for the day by listing their accomplishments and also rating themselves (1-10) for productivity. There is also a notes section for the teacher or student to make notes in.
I use this document to track my students during any long-term projects where I need to hold them accountable for less structured time when they are more self- or group-driven. Students must complete the "Goals" section on Monday and complete the rest of the chart as the week progresses. On Friday, they hand it in and I give them participation points for the week based partially on the chart.
