I used this in 3rd grade. My mom used this same form with 5th grade.
Fill in any behavior issues or incompletion of work and attach it to their graded work for the week and send home for parents to sign.
I also had a place to update where they were in completing their Accelerated Reader A.R. Goal each week. My class had the most students going to the A.R. celebration at the end of each 9 weeks and I think it had a lot to do with keeping parents informed each week with their progress. I am so glad I came up with this form to use. It was truly a life saver with communicating with parents and having documentation of our communication. It has 2 reports on one page. I am all about saving paper. So you can complete 2 reports on one page to save paper. I used this document every week all year long.
I’ve included 3 different versions of the report with the A.R. section changing in each. It has percentage, points, and one that does not show the A.R. section.
