I have been busy making forms for my class and students to track their progress when it occurred to me that a form like this would be really handy for me to have for myself too. This form was made with the intent to track progress within the Weight Watchers program. I made it to track weight loss up to 50 pounds but you could copy it and track another 50 toward a larger goal if you wanted to. There's also space to track progress by week and month which can be used for any year. In addition there's a space to mark off when you meet your 5% goal, 10% goal, lifetime, and any non-scale victories you want to record. It can be highlighted, colored, or you could even put a sticker over each achievement. I hope you like it!
