Product Description
This is just a simple worksheet to be used with students in speech to promote goal awareness and understanding, especially for students who need visual supports! I've used it for both students with Autism and students in multiple disabilities classrooms. I used BoardMaker to print pictures for each box that were individualized to the student. Here are a few examples of things that I've used in each category:

What I'm learning: 'requesting', 'following directions', making choices', 'participating', 'exchanging greetings'

Communication helpers: ears, eyes, hands, specific devices, PECS, pictures, low-tech switches

Who I like to communicate with: family, teacher, friends, therapist

You can have students who are able identify the things that they use to communicate and choose who they love to communicate with.
