This is a quick getting-to-know-you survey, suitable for a first day activity in English class. While brief, it allows for students to identify their interests in studying English. It also gives students the opportunity to be honest about their feelings toward English class, which can be an ice breaker for later dialogue. I use this survey to group students for later independent study options (reading, writing, researching). It also serves as a good source to find out what's "hot" in current teen literature, as students have the opportunity to discuss how they feel about recent independent reading.
