This 2-page PDF file includes a blank form and a completed sample to show students, if you wish. The sample is from my students’ study of Romeo & Juliet, showing that Juliet is definitely a Leo and Romeo’s probably a Gemini. The worksheet, though, is designed to work with ANY piece of literature.
