I copied this sheet on colored paper and then cut the stack down the middle lengthwise and kept them handy for each day. I made a small stack at the beginning of the year so they were always ready to go.
The design of this make-up work slip is that there is plenty of room to write down any assignments/tasks for each subject for that day. The slip is organized with a place for Math, Science, Social Studies, and Language Arts & Reading.
Depending on your grade level, you could have a student "volunteer" carry this around for the day writing down the assignments and collecting work for the absent student. The great thing about this is you are letting a student help out with classroom jobs and it is very little work on the part of the teacher. It is also an easy structure for a teacher to complete at some point during the day.
I would always ensure that this slip was attached to any work from the day and then placed on that student's desk at the end of the day. I wanted this to be the first thing that student sees when they come to class the next day.
