This is a simple foldable that can be used in any content area to discuss the five important questions. I first used it with an art project inspired by Picasso then did a mini biography on him telling who he was, what he did, when he did it, where he did it and why he is famous. Use it however you would like. This makes a great assessment of what students learned in a lesson. Paste it onto a piece of construction paper with a title above to make a nice project.
Copy it front to back. Cut the large rectangle out then fold in half. Next, cut the lines between the questions to the fold. Open each section and answer the question on the lines provided.
