As an SLP, you probably encounter this question quite often. You now have an answer, but in mini-book format! Use these mini-books when you first start seeing a student or when you are experiencing resistance about attending speech therapy sessions. These are appropriate for students 6th-12th grade. They are in black and white; the pages are designed so that the student can write in information based on why he/she sees you. This can help make the student accountable. Speech therapy contracts (one for articulation, language, and fluency) are included to help make the student accountable. These can be completed as part of transition plans for IEPs and would be a great way to involve families and other staff.

-Articulation (pg. 3-6)
-Fluency (pg. 7-10)
-Language (pg. 11-19: Version 1 – choose the pages that best suit your student, pg. 20-23: Version 2 – print and fill in as many pg. 20 as needed)
-AAC (pg. 24-27): designed to be read to the AAC user who can answer with his/her device; SLP can check off answers
-Life Skills (pg. 28-31): designed with graphics, so student can circle pictures that match the skills he/she is working on, blank page included
-Speech Therapy Contracts (Articulation pg. 32, Language pg. 33, Fluency pg. 34)
