This is a booklet I made for my fourth graders to teach them how to do story problems in an organized way. (it focuses on multi-digit addition and subtraction problems), but the concept of how to do the story problem can apply to all types of story problems.
I found this to be good for a whole class lesson, partner practice, or self starts.
