Use this double-sided cover page for your 3rd-5th graders' final writing drafts. This can be used as-is for both fiction and expository writing.
One side asks the writer to answer a few simple questions helping them self-reflect on their strengths, areas for growth, and the most helpful processes used while writing the piece.
The other (optional) side leaves space for fellow classmates to write comments to the author after reading each others' work. (Note: Students can be encouraged to only leave positive comments or observations.) After each writer completes the self-reflection side, I hang the students' stories on the bulletin board with the "comments for the author" cover page attached. As the students read each others' stories, they leave encouraging and positive feedback for the author before hanging the story back up.
I have left this as a word document so it can be edited to include any classroom-specific language you may use.
