Having searched for a writer's workshop record that was user friendly, I decided to make my own. Students keep a writer's journal in my classroom for collecting "seeds" for writing (prompts, brainstorming, quotes from books, etc.). They are required to write and turn in 2 pieces of writing to me each nine weeks. These pieces are of their own choosing. I conference with students throughout the process and found this form makes it easy to keep up and makes the process faster. It is two pages, which I print double sided, and keep in a binder. I print one for each student. There is a checklist for what students are working on and what point they are at, along with a notes section to jot down what was discussed at each conference.
